Belle Babes Registration Form - February 6, 2010

Registration is due by February 1, 2010. Walk in registration is accepted, but uniform sizes are done on a first come, first serve basis. We will be wearing uniforms at the Basketball game this time.

Registration is $50.00. Registration is not complete and your spot is not guaranteed until payment is received. You can mail your payment to the Apache Belle office or call the Apache Belle office at 903-510-2244 with your credit card information.

I certify that the above applicant has my permission to participate in the Belle Babes Workshop and performances. I authorize the director of the Apache Belles to act for me according to her best judgment in any emergency requiring medical attention. I hereby waive and release the Belle Babes Workshop and performance from liability for injury. I know of no mental or physical problems which affect my child's ability to safely participate in this workshop. I further certify that the above mentioned person has medical insurance in case of emergency.
